Although parking was a pain, the staff was very nice which made up for that. The facility had a pleasant atmosphere. Staffing really cared about the well being of the patients and the patients families.

She’s in a private room, which was designed for two people, so she has a private room by herself, it’s a big room. The bathroom is linked to another room, so that’s not a good thing. The nurses are very friendly, and they do a lot of different things with them. I think they have games in the main room, play games or cards, and they have a schedule on the wall on each room, so she can go to it if she wants. My mom is in the secure unit; it’s not really nice. The staff at Liberty Nursing Center of Toledo are all part of the care team for the residents, that from the janitor to the director.
They also note that Seasons offers in-room kitchenettes and kitchens, resident parking, a beauty and barber shop, residential parking spaces, garden and patio, and a pool to relax in. They also followed the directives the doctors gave. You see the people waxing the floors and cleaning them. There’s no bad smells like most nursing homes have.
The day nurse was not invited to my care conference because of staff infighting between social worker and nurse. I was not given the balance of my medications upon discharge and my departure was then delayed 2 hours while they got the undispensed prescribed meds together for me. My poor review is because the inability to dispense the right medications and not dispense the wrong medications and keep to an semblance of the dispensing schedule. I was cognizant enough to notice; unfortunately many residents were not. The dispensing records are all hand written in paper logs; nothing is scanned or automated, and these procedures were very inefficient for the nurses.

Warren County had 16 nursing homes, as of April 1, 2017. Of those, 13 homes, or 81%, are rated above average, meaning they have a four- or five-star rating. Van Wert County had three nursing homes, as of April 1, 2017. Of those, one home, or 33%, is rated above average, meaning it has a four- or five-star rating. Union County had three nursing homes, as of April 1, 2017.

Of those, two homes, or 67%, are rated above average, meaning they have a four- or five-star rating. Portage County had nine nursing homes, as of April 1, 2017. Richland County had 10 nursing homes, as of April 1, 2017. Of those, four homes, or 40%, are rated above average, meaning they have a four- or five-star rating. Putnam County had five nursing homes, as of April 1, 2017.
Some of the nurses at the beginning were very friendly and paid wonderful attention to my grandmother. Once her stay hit the week mark, the treatment began to deteriorate. After her stroke, the doctor ordered physical therapy to help strengthen her affected side.
They have very tiny small portions of food too. The place is very dirty and they don’t keep it clean. I’ll give them credit for their therapy department which is very good. Westminster-thurber Community is conveniently located near Interstate 670 in Columbus, Ohio, just a short walk from Goodale Park and only blocks from a variety of casual dining restaurants. Family members are impressed with the facility’s modern, cozy look and convenient location, noting its proximity to theater, museums and the symphony.
